Intake Forms

If it is your first visit, you may either download and fill out your respective intake form below or come five minutes early to fill it out. Click on the intake form that you need below.

Appointment Hours

• Thursday: 9:30am, 11:00am, 3:30pm, and 5:00pm
• Friday: 9:30am, 11:00am, 2:00pm, and 3:30pm
• Two Saturdays per month: 9:30am, 11:00am, 12:30pm

What to Wear

Please wear loose-fitted, comfortable clothing (such as yoga clothes or sweat-pants).

Session Length and Rates

• Craniosacral Therapy sessions are 75 minutes, for $90.
• Body-Centered Counseling sessions are 75 minutes, for $65.
• Craniosacral Therapy for children is $40. Please allow 30-45 minutes.
• Only checks and cash are accepted.
• If an appointment is missed without 24 hours of notice, the full price of the session will be expected (unless there is a sickness or emergency).

Insurance Billing

Insurance billing for motor vehicle accidents is available with a written prescription from your primary health care provider

Please come inside and wait in either the entry-way room or the adjacent waiting room (please help yourself to tea--which can be found in the large cupboard). Jeremy will receive you in either of these rooms for your session.
